How to schedule a FedEx collection?
To schedule a collection and generate the shipping documents, please follow the steps below.
Schedule Pickup and Generate Documents
Visit our Shipping Page to generate the shipping documents and to schedule your FedEx pickup.
Simply enable pop-ups in your browser window when clicking "Get Label" to ensure a smooth generation of your documents.
Generate Shipping Documents
Print three copies of the digital shipping documents provided (AirwayBill and Commercial Invoice). Ensure the Commercial Invoices are signed at the bottom.
Schedule a Pickup
Schedule a pickup time that suits your schedule, allowing FedEx a 3-4 hour window for collection.
The blood collection kit serial number can be found in the lid of the box or on the outside.
Seal the FedEx Clinical Pack and keep the shipping document outside the Clinical Pack ready to handover to the FedEx courier.
Guardant Health Portal
Our Guardant Health Portal will allow HCPs to access patient reports and check the test status of samples sent into our laboratory HCPs. They can also invite other internal colleagues to view the results.
How to activate your portal account:
Please visit the Guardant Health Portal. Enter your username (email) and click "send activation link", to receive a verification link in your personal inbox.
To confirm your identity, open the email and click on the verification link contained. This will bring you to your Guardant Health Portal to create a password.
How to login to your portal account
Please visit the Guardant Health Portal and enter your credentials (username and password). If you forgot your password please click on "Forgot your password?" and you can reset it from there.
These are the password requirements:
Password requirements:
- 12 characters
- At least one UPPERCASE
- At least one lowercase
- A number (123)
- No parts of your username
- Your password cannot be any
of your last 4 passwords
